The difference between Zebra QL420 and QL420 Plus printhead
There's no difference in the printhead itself - the QL-420 and the QL-420 Plus use the same 203-dpi, 4-inch thermal printhead assemblies (common OEM part numbers: RK17735-004 / RK18253-1). Replacement printheads and compatible aftermarket heads are sold for both models as the same part.
What that means:
Print quality / resolution: both use a 203 dpi printhead, so print quality and dot density are identical.
Form factor / fit: the printhead assembly (connector, mounting points, width) is the same - a replacement listed for RK17735-004/RK18253-1 will fit either QL-420 or QL-420 Plus.
When they differ:
the printers differ in features (the "Plus" is an updated model with rubberized over-molding, optional LCD/battery/comm options, etc.), but those are chassis/firmware/hardware differences - not the printhead.
